Thank you for participating in the Spectrum Health Medical Group Sports Medicine
athletic pre-participation physical examination day. This is a service provided to you as part of the comprehensive sports medicine program offered in your school.
Our goal is to provide your son and/or daughter an efficient, cost effective, multi-disciplinary medical examination in order to help ensure greater safety in future athletic participation. While no medical exam can completely eliminate all risks associated with participation in sports, we aim to provide a more thorough physical exam.
This examination does not and should not take the place of a yearly well-check by your child’s health care provider!
To help ensure a smooth process, we ask that parents complete the MHSAA
Physical Form accurately and completely. This form must then accompany your son/daughter on the physical day. We will not be able to complete the physical exam without this form completed with signed parental consent. After the physical exam process is complete, the forms will be collected in the office and returned to the schools. In the event that your child is not cleared for participation as a result of findings during their physical exam by our physicians, we will communicate with you directly to help direct any necessary follow up care.

Rockford Public Schools require all athletes to have a physical on file before a student may try out or participate in a sport. You may pick up a physical form in the main office. You must return the completed form, with physician's approval, to the main office prior to try-outs. All sports physicals must be dated after April 15, 2015 to be valid for this school year.
Academic Eligibility Policy for 7th and 8th Grade:
1. An athlete must pass at least five classes with a minimum of three C's.
2. Ineligible students will serve a two-week period of athletic ineligibility. If grades have met the standards, the athlete will then be allowed to compete. If the athlete has not attained minimum standards after the initial two-week period, grades will be checked on a weekly basis thereafter to determine eligibility/ineligibility.
3. If ineligibility takes place during the in-season, the athlete must still attend all practices and team functions per the coaching staff.
4. Semester grades (1st and 2nd) take precedent over nine-week grades (2nd and 4th) in determining eligibility.
Middle School - A Team Sports $90.00 per sport
Middle School - B Team Sports $90.00 per sport
Family Maximum - $350.00 per family
Practices generally take place after school at 2:30 pm. The coach will provide you with a practice and game schedule. We will provide transportation to and from away athletic events. For contests at North Rockford Middle School, transportation will be provided to the contest only. Athletic events generally start at 4:00 pm.
