Rockford Public Schools is pleased to welcome you to the Kindergarten/Developmental Kindergarten enrollment process. If you are enrolling a new K/DK student, please be aware that students must be five years old by September 1st (students may attend K/DK with an Age Waiver if they turn five between September 1st and December 1st). We understand that this is a monumental time in a child’s life and we eagerly welcome our future K/DK students.
Begin the enrollment process in a few easy steps!
Step 1: Complete the Enrollment Form
Step 2: Bring the following required documentation to the Administration Building at 350 N. Main St, Rockford, MI on Wednesdays and Fridays between 9:00am and 3:00pm (You may call 616-863-6320 with questions):
• Original certified copy of the student’s birth certificate
• Immunization records
• Hearing/Vision Screening results
• Medical Information Form
• Proof of Residency (any two of the following): property deed/tax bill, utility bill or lease/rental agreement
Step 3: District recommendations for screening (see chart):
|Your child is five years old on or before April 30, 2017||*District strongly encourages kindergarten placement|
|Your child’s birth date is between May 1st and August 31st||*District recommends screening for placement unless your child is currently in DK.|
|Your child’s birth date is between September 1st and December 1st||*Age Waiver is required
*District strongly encourages Developmental Kindergarten placement
*If screening is recommended as indicated above please sign up here. Please note: This is a district–wide summer screening which will be held at Meadow Ridge Elementary.
Step 4: Please complete this step only if you have a K/DK student who will need transportation to or from a daycare: Transportation Form
