New Student Registration
To begin the enrollment process, print and complete the following forms:
• Athletic Form *required for students in grades 10-12 only

Summer Central Registration – June 11-August 17
During the summer break, new student registration will take place at Rockford High School Monday through Thursday from 10am–2pm and Fridays 8am–11am. Appointments are recommended. Please feel free to call 616-863-6031 ext. 7010 with any questions. A parent/guardian is expected to bring in the required documentation to the main entrance at Rockford High School 4100 Kroes St. NE, Rockford MI 49341. We look forward to welcoming you!
Note: The office will be closed Wed. July 4 – Fri. July 6 for the holiday recess.
Required Enrollment Documentation
In order to enroll your child(ren) into Rockford Public Schools, we will need the following:
Proof of residency in Rockford with parent/guardian’s name and residential street address (no P.O. boxes):
You must physically reside within the Rockford Public School District boundaries to be considered a resident. Two items of proof must be provided. Acceptable proof includes 1) your property tax bill AND one utility bill; 2) a current signed rental agreement showing parent/guardian name, address, and contact telephone number; 3) other acceptable proof as determined by district staff. If you do not reside in our district, you may qualify for admission through schools of choice.
Original birth certificate.
The Michigan Missing Children’s Act (MCL 380.1135) requires that you provide a certified copy of the child’s birth certificate when you are enrolling in school. We will return the certified birth certificate to you. If you do not meet this requirement within 30 days, the school must notify local law enforcement.
Complete immunization records.
If your child is entering a Michigan school for the first time, please have your health care provider complete the immunization portion of the Health Appraisal form, or provide the school with a record showing that your child has received all of the required immunizations. The Health Appraisal form or other proof of immunizations must be returned to the school your child will be attending on or before the first day your child attends school.
