Bring the following required documentation to this event:

• Original Birth Certificate – We will return the certificate to you
• Immunization records
• Hearing/Vision Screening results
• Proof of Residency (any Two of the following): Property deed/tax bill, utility bill or lease/rental agreement
• Parent Questionnaire (PDF)
• Home Language Survey (PDF)
